Tranexamic acid is an antifibrinolytic agent used to reduce peri-operative blood loss. This randomized study evaluated its effectiveness and thrombotic risk in patients with femoral diaphyseal fractures treated with SIGN intramedullary nailing. Eighty patients were assigned to receive either tranexamic acid or normal saline. Calculated blood loss and reduction in haematocrit were significantly lower in the tranexamic acid group, and hospital stay was shorter. However, transfusion rates were not significantly different between groups. D-dimer monitoring showed no significant increase in thrombotic events, though postoperative changes persisted longer in the tranexamic acid group. Tranexamic acid effectively reduces peri-operative blood loss and supports early recovery without increasing thrombotic risk.
